QMP Ultrasonic Rhinoplasty Using a Full Open Approach — piezo bone work demonstrated case by case
QMP Ultrasonic Rhinoplasty Using a Full Open Approach: A Revolution in Rhinoplasty puts you beside Olivier Gerbault for roughly nine hours of operating as he replaces the chisel-and-fracture routine with piezoelectric saws, rasps and scrapers. Because ultrasonic instruments cut bone and rigid cartilage while sparing skin, mucosa, vessels and soft cartilage, he can expose the entire bony vault, reshape it under direct vision and fix fragments with sutures. Six complete operations show where the technique earns its place.

What you’ll be able to do
- Plan an extended subperiosteal undermining that reveals the whole bony vault before any cut
- Perform controlled piezo osteotomies, complete, partial or asymmetric, and position each bone deliberately
- Sculpt and smooth the dorsum with rhinosculpture rasps so the keystone feels untouched
- Remove a hump incrementally while protecting the upper lateral cartilages and avoiding step-offs
- Stabilise mobilised bones with suture osteosynthesis in primary and revision cases
- Carry out an ultrasonic septoplasty and a safer piezo rib graft harvest
Inside the video
Why ultrasonic rhinoplasty changes bone management
Gerbault, who has been refining piezo instruments and techniques for the nose since 2013, explains the shift from blind fractures to open, visible bone reshaping. Mechanical osteotomes often send uncontrolled fracture lines through the nasal bones; the piezo tip does not, which is what makes precise mobilisation possible.
Four primary noses with different problems
Two male patients, one with a tension nose and wide vault, the other with a wide asymmetric nose on an asymmetric face, receive complete or asymmetric osteotomies with septal work. Two female patients show the lighter end of the spectrum: piezo hump reduction with partial osteotomies, and a pure rhinosculpture paired with tip plasty.
Revision and the twisted nose
A secondary case with no tip support and valve collapse demonstrates rib cartilage harvested with the piezo device and used to rebuild septum, tip and valves. The final operation tackles a deviated nose with a dorsal hump using full osteotomies and ultrasonic septoplasty.
Built for
- Rhinoplasty and facial plastic surgeonsA structured view of how piezo tools change every bony step of the operation.
- Otolaryngologists doing septorhinoplastyUseful for the septoplasty and bony-vault segments, including long intact bony septum harvest.
- Surgeons in trainingWatching complete cases shows the sequence and pacing that a lecture cannot.
Operations shown
Surgical cases 6 cases
- Primary male — overprojected nose (tension nose), wide bony vault and wide asymmetric tip, septal deviation: complete osteotomies and ultrasonic septoplasty
- Primary male — wide, asymmetric nose on an asymmetric face, severe septal deviation, bony hump: asymmetric osteotomies, bone reshaping and ultrasonic septoplasty
- Primary female — nasal hump, overprojected drooping nasal tip: piezo hump reduction, partial osteotomies and tip reshaping
- Primary female — hump reduction on a slightly too wide bony vault, overprojected tip: ultrasonic rhinosculpture with tip plasty
- Secondary female — asymmetric nose with no tip support, valve dysfunction: piezo rib graft harvest for septal, tip and valve reconstruction
- Primary male — twisted nose with dorsal hump, septal deviation: complete osteotomies and ultrasonic septoplasty

How recent is the Gerbault footage?
The programme was released in 2018. Piezo instrument designs have evolved since, but the operative principles demonstrated here remain the core of the method.
Does a written text or chapter list come with the videos?
No companion book is included. Content is organised around the six operations; exact file names, sizes and resolution were not specified.
Do I get CME credit or a certificate?
No. Clinicapedia provides the educational content only — no CME or CE credits, MOC points or certificates of completion are included with this product.
