Facial feminization surgery in seven full cases — James Bradley’s 3D-planned approach to brow, nose, jaw, lips and neck
Advanced Techniques in Facial Gender Surgery: 2025 Masterclass follows Dr. James P. Bradley and his operating team through seven facial feminization patients aged 24 to 37, in 18 h 30 min of operative video. Each case starts with history, prior procedures and 3D skeletal and soft-tissue simulation, then proceeds through the brow, nose, midface, lips, mandible and neck, including primary and revision operations.

Inside the course
Primary full-face cases
Patients aged 24, 25, 27 and 34 undergo combined brow, nasal, lip, mandibular and neck work. The 24-year-old had three years of hormone therapy before surgery and receives extra fat grafting; the longest case, at 3 h 39 min, adds midface volume and hairline advancement for a patient who began transitioning ten years earlier.
Revisions and complicated histories
A 37-year-old with prior vaginoplasty, cheek and chin implants and silicone-injected neck tissue; a 28-year-old needing correction of cheek implants plus revision rhinoplasty; and a 30-year-old whose secondary operation includes tracheal shave and fat grafting.
How Bradley runs the day
Throughout, he explains his preferred order of steps, instrument choice and how a coordinated team keeps long multi-region operations efficient.
