Hands‑On Open Rhinoplasty Masterclass: Achieving Subtle, Primary Hybrid Results for a Naturally Refined Nose follows Enrico Robotti through six complete operations, each lasting roughly three to almost five hours. Five are primary open rhinoseptoplasties that blend dorsal preservation with structural tip work; the bonus sixth is a rib-graft revision for a saddle nose. Piezo and conventional powered instruments are used together for osteotomy and osteoplasty in every case.

What you’ll be able to do
- Decide when a full let-down, a cartilaginous push-down or a modified push-down that includes part of the bony cap suits the dorsum
- Expose tip and dorsum in subperichondrial and subperiosteal planes and restore the ligaments afterwards
- Apply three-point compartmentalization and a septal fusion sling to control swelling and shape
- Combine piezo and power tools for osteotomy, osteoplasty and piezo turbinoplasty
- Correct alar retraction with a conchal composite graft and place an intraoral chin implant
- Rebuild a saddle nose with rib cartilage and an SPLF dorsal graft
Inside the course
Four primary hybrid cases
Patients aged 21 to 49 present with humps, deviation, bulky or drooping tips and in several cases a blocked airway. Robotti uses a modified dorsal split with high- or low-strip let-down or push-down, then shapes the tip with lateral crural steal, LLC turn-under or slide-under, septal extension grafts, spacer struts, alar contour grafts and, in one case, diced cartilage.
Reconstruction after neurofibroma excision
The longest video, 4 h 41 min, treats a 25-year-old man left with deformity after childhood neurofibromatosis surgery. It merges primary and revision thinking: perichondrial–periosteal flaps, removal of residual tumour, lateral crus restoration, bilateral spreaders and a conchal composite graft for a retracted ala.
Chin and revision work
One primary patient also receives an anatomic silicone chin implant through the mouth. The bonus case harvests rib, rebuilds the L-strut with layered grafts and extended spreaders, lengthens lateral crura and augments the dorsum with an SPLF graft and rib plaster.

No. All six lessons are operative recordings; the teaching comes from Robotti’s commentary while he works, not from slide presentations.
