Endodontics under the electron microscope — David Jaramillo on biofilm, histology and irrigation evidence
UTHealth Houston residency director David Jaramillo uses scanning electron and confocal microscopy images to show what actually happens inside a canal in Deep Dive Endodontics: Evidence, Histology, and Practice. Four long lessons (about 7 hours) connect caries and pulp histology, bacterial detection, biofilm in the apical third and a detailed comparison of today’s irrigation and activation systems to everyday treatment choices.

What you’ll be able to do
- Read SEM and confocal images of dentine, biofilm and sealer interfaces
- Relate the depth of a carious lesion to the pulp’s histological response
- Recognise and treat intraoral sinus tracts and gutta-percha extrusion
- Place, use and fully remove calcium hydroxide as an interappointment dressing
- Compare ultrasonic, PIPS, EndoVac and GentleWave irrigation on the evidence
Inside the course
Caries, pulp and diagnosis
Dens evaginatus, diagnosis of pulpal and periapical disease, CBCT detection of cracks and microscopy of early, moderate and deep lesions lead into biofilm inside tubules and canals and how to obturate lateral canals in three dimensions.
Bacteria inside and outside the root
Jaramillo explains the development of periradicular disease, uses LIVE/DEAD staining to judge bacterial viability, and shows specimens that help separate conditions with similar clinical signs.
Microbiology across treatment
Samples taken before, during and after instrumentation illustrate acute versus chronic infection, host defence against biofilm and strategies to eliminate it.
Irrigation in depth
The longest lesson (2 h 20 min) covers NaOCl, CHX and EDTA, smear layer removal, laminar versus turbulent flow, activation devices on the market and complete irrigation protocols backed by current research.
