Hyperdivergent Class II, MARPE and aligner-TAD combinations — difficult orthodontic cases in 7 lessons
Four orthodontists who publish and lecture internationally — Benedict Wilmes, Kee Joon Lee, Armando Dias da Silva and Daniel Souza Pinto Ramos — share their approaches to the cases most practices refer out in Extreme in orthodontics: treatment of complex cases. Across 9 h 25 min you get total arch intrusion for long faces, miniscrew-assisted and surgical expansion, aligner biomechanics, skeletal anchorage and orthognathic planning with clear trays.

What you’ll be able to do
- Diagnose vertical excess from the soft tissue profile and plan total arch intrusion with miniscrews
- Classify the midpalatal suture on CBCT and choose between MARPE, SARPE and BAME
- Build a force system and attachments in aligner planning software with sensible overcorrection
- Place infrazygomatic and buccal shelf TADs with a digital, guided workflow
- Stage ortho-orthognathic cases with aligners from diagnosis to postoperative review
- Distalise and mesialise molars with Beneslider and Mesiaslider mechanics
Inside the course
Long faces without orthopaedics
Two lessons treat the hyperdivergent Class II patient: cephalometric and profile analysis, rotation of the occlusal plane around the centre of resistance, miniscrew placement and extraction space closure when incisors are retroclined.
Transverse problems in adults
The longest session (about 2.5 hours) covers tomographic transverse analysis, suture classification, device types and work protocols, and anatomy such as palatine artery and nasopalatine canal variations, with crossbite, open bite, Class II, Class III and impacted canine cases.
Aligners pushed further
Rules for diastemas, crowding, IPR, expansion, extraction, open and deep bites, then DSD-guided TAD planning and extra-alveolar anchorage used together with clear trays.
Surgery and distalisation
Decision algorithms for skeletal Class II and III orthognathic cases lead into palatal microimplants, two-step distalisation, asymmetry correction and long-term results.
Built for
- Orthodontists using TADsConcrete biomechanics for intrusion, expansion and molar movement.
- Aligner providersPlanning rules that expand what clear trays can do in difficult cases.
- Teams planning orthognathic surgeryFacially driven digital workflows shared between surgeon and orthodontist.

Is lesson 5 really listed as 1 h 97 min?
That is how the running time appears in the lesson list; it is probably a typing error for roughly 1 h 37 min or 2 h 37 min.
Which faculty member pioneered MARPE?
Kee Joon Lee of Yonsei University is credited with first demonstrating the miniscrew-assisted rapid palatal expander in adults.
Do I get CME credit or a certificate?
No. Clinicapedia provides the educational content only — no CME or CE credits, MOC points or certificates of completion are included with this product.
