Iñaki Gamborena’s eight-step immediate implant protocol — provisional first, guided placement, tuberosity CTG and 15-year follow-ups
In One-Protocol Approach: Surgery + Restoration for Fresh Socket Immediate Implants, Iñaki Gamborena of San Sebastián, affiliate professor at the University of Washington, shows how he fuses surgical, laboratory and restorative steps into a single workflow for anterior extraction sockets. Fifteen lectures (14 h 25 min) combine theory with surgical footage from his own clinic, covering socket types 1 to 3, adjacent implants and an all-at-once definitive crown concept.

What you’ll be able to do
- Place a provisional on the failing tooth before extraction and copy its emergence profile to the implant
- Extract atraumatically without instrumenting between root and crestal bone
- Plan the implant in DTX Studio with SmartFusion and transfer it to a printed guide and custom abutment
- Reach 35 Ncm insertion torque and pack the gap with 3D bone grafting
- Harvest and place tuberosity connective tissue for long-term soft tissue stability
- Adapt the approach to Elian type 2 and type 3 sockets and to adjacent implants
Inside the course
Why soft tissue decides the result
The opening lectures examine esthetic implant failures, compare palate and tuberosity as graft donor sites — the tuberosity yielding denser, less fatty tissue that shrinks less — and trace the evolution of the concept with long-term cases.
The eight steps in order
Provisionalisation, atraumatic extraction, guided placement, primary stability, bone packing, custom abutment delivery, provisional reline and tuberosity graft are each explained, from estimating graft volume from abutment collapse to briefing the technician on margin location.
Digital workflow and live cases
A surgical video follows the chain from PVS impression poured in epoxy and full-contour wax-up to STL export, guide printing and transfer of implant head orientation. Further cases show a lateral incisor with adjacent pontic on Ti Ultra implants.
Harder sockets and long horizons
Type 2 sockets are rebuilt, including with a multilayer technique; type 3 cases need deeper placement and a transmucosal abutment. Adjacent-implant cases documented from 2006 to 2021 and the 2016 all-at-once concept with a pre-made zirconia crown close the course.

Before you buy
Is the protocol tied to specific software or implants?
The digital demonstration uses DTX Studio Implant and one case uses Nobel Ti Ultra implants, but the eight principles are presented as system-independent.
Is it suitable for someone new to immediate implants?
It assumes surgical experience; the all-at-once concept in particular is described as technique-sensitive and reliant on close clinician–technician communication.
Do I get CME credit or a certificate?
No. Clinicapedia provides the educational content only — no CME or CE credits, MOC points or certificates of completion are included with this product.
