Diagnosing and restoring severe tooth wear with Kinzer and Rouse — etiology, vertical dimension and phased treatment
Seattle prosthodontist Gregg Kinzer and San Antonio restorative dentist Jeff Rouse deliver Treating the Worn Dentition as a single 11 h 34 min recording with an accompanying PDF. They set out how to recognise the cause behind each wear pattern, decide whether the vertical dimension has really changed, and plan treatment that may involve orthodontics, periodontal or orthognathic surgery before any restoration.

What you’ll be able to do
- Link each wear pattern to its etiology and use that to predict prognosis
- Assess how wear has shifted tooth position, gingival and papilla levels and occlusion
- Decide whether vertical dimension was lost and set a new one when needed
- Judge when orthodontics, crown lengthening or orthognathic surgery should precede restoration
- Use composite trial therapy to test a plan and phase costs over time
- Choose anterior and posterior materials and talk honestly with patients about longevity
Inside the course
Why wear cases feel risky
Heavy wear often makes dentists hesitate or refer. The presenters argue that most of the uncertainty disappears once the cause, the altered tooth and tissue positions and the true vertical dimension have been worked out.
A repeatable planning sequence
They then lay out a stepwise approach to planning, including how to compare orthodontic, periodontal and surgical routes for moving teeth or tissue into a restorable position.
Testing and phasing with composite
Direct composite is presented as a way to trial a new occlusion and appearance, and to spread severe cases over years while keeping the final ceramic plan open.
The presenters
Kinzer maintains a Seattle practice limited to comprehensive restorative and esthetic dentistry; Rouse practised alongside him and Frank Spear and co-wrote a 2016 textbook on global diagnosis.
