Moving teeth safely through thin, compromised alveolar bone — aligners, Damon mechanics and intrusion for adult periodontal patients
Four orthodontists — Trevor Nichols, Bakr Rabie, Kamy Malekian and Rainer-Reginald Miethke — share how they treat adults whose periodontium can no longer tolerate routine mechanics in Orthodontic Treatment in Alveolar Bone Bone Deficiencies. Seven lessons, about 11.5 hours in total, cover light-force principles, space opening ahead of implants, bodily movement into extraction sites, relative versus true intrusion, clear aligner protocols and the Damon system in grown-up dentitions.

What you’ll be able to do
- Plan lower molar distalization with aligners for crowded Class III cases without extractions
- Choose between incisor intrusion and mandibular rotation when correcting an adult deep bite
- Control the vertical dimension in high-angle open bite using segmental mechanics and microimplants
- Adapt attachment design and retention to patients with reduced periodontal support
- Apply Damon and AdvanSync Herbst options to non-surgical adult Class II and Class III correction
Inside the course
Aligners as a periodontal-friendly tool
The first three lessons build an Invisalign workflow for patients with bone loss: ClinCheck set-up around four treatment goals, the 1/2 versus 1/3 staging protocols, an aesthetic-first start that keeps patients engaged, and canine extrusion. A severe non-extraction Class III crowding case shows proclination, expansion and distalization balanced in one plan, with long-term interdisciplinary follow-up.
Vertical problems in adults
One lesson dissects the Class II division 2 deep bite, from causes of incisor overeruption to reverse-curve wires, live wire bending and ways to limit root resorption. Another tackles long-face open bites: when to extract which tooth, how transseptal fibres affect stability, lateral microimplant intrusion, autorotation and when surgery becomes the honest answer.
Fixed appliances versus trays
A Damon-based lesson covers smile design, laser soft-tissue contouring and tooth reshaping during treatment. The closing lecture weighs braces, lingual systems and aligners for older patients, lists ten planning considerations and reviews common adverse events with trays.
The teaching panel
Rabie, formerly a professor at the University of Hong Kong, is known for graduate orthodontic teaching; Malekian has treated more than 2,400 Invisalign patients in France; Nichols practises in Arizona; Miethke holds honorary roles in several national orthodontic societies.
Built for
- Orthodontists treating adultsMost of the cases here involve bone loss, implants or long faces rather than teenagers.
- Clinicians moving into alignersClear ClinCheck logic for patients whose gums limit force levels.
- Periodontists in shared casesInsight into what orthodontic movement can offer before implant or regenerative work.

Before you buy
Is the course only about aligners?
No. Aligners dominate four lessons, but deep bite and high-angle lessons rely on wires, segmental mechanics, microimplants and the Herbst appliance, and one lesson is built on Damon brackets.
How much overlap is there between the primer and lesson two?
Partly. The short primer and the two-hour lesson share several topics, such as molar distalization and the 1/2 versus 1/3 protocols; the longer one adds retention, implant patients and combined Class III open bite cases.
Do I get CME credit or a certificate?
No. Clinicapedia provides the educational content only — no CME or CE credits, MOC points or certificates of completion are included with this product.
